Pulpal diagnosis: asymptomatic irreversible pulpitis (caries exposure)
Periapical diagnosis: normal periapical tissue
Noted a C-shaped canal morphology both clinically and CBCT. The ishmus, however, appeared extremely narrow, and the buccal concavity between the mesial and distal canal appeared so pronunced that aggressive instrumentation of the area will likely result in strip perforation. The final radiograph shows the minimally instrumented isthmus area is now occupied by the sealer in the coronal and middle third.
